Special Report of Late Contribution (EB-3) reports late contributions that are $500 or greater, either as a single contribution or as a cumulative total from any single source during the special reporting period. This report is filed by candidate committees, political committees or individuals.

Special Report of Late Contribution for Conduits (EB-4) reports late contributions that are $500 or greater, either as a single contribution or as a cumulative total from any single conduit during the special reporting period. This report is only filed by conduits.

Oath for Committees and Individuals Making Independent Disbursements (EB-6) must be filed before independent disbursements are made. This form states who will be making the expenditures and which clearly identified candidate the filer will be either supporting or opposing with the expenditures. The oath is valid for the current calendar year.

Report of Independent Disbursements (EB-7) is the report of late independent expenditures. This report is required to be filed if any expenditures of more than $20 cumulatively are made by any committee or individual to advocate the election or defeat of any candidate when that expenditure is made without the candidate's authorization or cooperation. The committee or individual making the expenditure is required to file this report within 24 hours of making the disbursement during the special reporting period.
